Patient Referrals / Communications
Referral for all hospice services can be made by healthcare professionals

Referrals for hospice inpatients, outpatients, interventional pain team, wellbeing services and Wirral Community Specialist Palliative Care Team services, should be completed by healthcare professionals on our joint referral form at  www.wirralhospice.org/spcreferral and sent to wchc.csps@nhs.net

Referrals for Hospice at Home services can be found by clicking Hospice at Home, following the links and send to wih-tr.ReferralsWirralHospice@nhs.net

Advice and Information

Call 0151 334 2778 Monday to Friday 9am to 5pm for access to all of our care and support services.

Advice outside of these times can be accessed 24/7 via the Wirral Palliative Care Services and Advice Line on 0151 343 9529
