A. Specialist Palliative Care
Please download and complete the new Wirral Specialist Palliative Care Referral Form using the Specialist Palliative Crib information sheet to guide you. Then please attach to an email and send to wchc.csps@nhs.net
B. Hospice at Home (Home support visits)
Referrals to Hospice at Home home support visits, delivered by Wirral Hospice St John’s and Marie Curie nursing services, should be made by downloading and completing this Hospice at Home referral form and then attaching it to an email addressed to our NHS email/inbox: wih-tr.ReferralsWirralHospice@nhs.net
NB: Please DO NOT use info@wirralhospice.org email address for referrals or correspondence about patients.
