Hospice at Home

People can access our Hospice at Home service following a referral from their GP, a community nurse or other medical consultant. (The information for the referral process for all health professionals can be found here).

Hospice at Home enables people with life-limiting illnesses to remain in the comfort of their own home with help from Wirral Hospice St John’s.

There are two aspects to the service which are home support visits and home personal care services. These distinctions are described below.

Home Support Visits

Supporting families and other carers to take a break is a key part of our support.

We aim to complement care already being provided by family members and other carers and to help reassure people that their loved ones are in safe hands.

We can be around for a 3-hour day sit or full overnight visits to allow loved ones to get a little time for everyday tasks or for a good night’s rest.

These support visits are provided by Wirral Hospice in partnership with Marie Curie and are available 365 days a year.

Home Personal Care Visits

Patients who are likely to be within the last weeks or month of their life may want to stay at home, or get home from hospital.

We support people with personal care, meal preparation, nutrition, continence care, monitoring any other health conditions, concerns or changes.

Visits are normally between 30 and 60 minutes long.

For our personal care service we aim to respond within two hours of receiving a referral and is available during daytime hours 365 days a year.

Feedback and Contact

Hospice at Home is available, free of charge, to anyone with a life-limiting illness who is registered with a Wirral GP.
