Statement: Care Plans
When patients are admitted to our inpatient ward they are always assessed on an individualised basis and, in discussion with them and those close to them, an appropriate care plan is made that reflects their individual needs.
This care plan is reviewed frequently as the patient’s condition alters, including when it is felt by the clinical team, patients and or those close to them that the patient is dying, care plans are reviewed to make sure they continue to best meet the patient’s needs.
Medications are provided for the control of distressing symptoms; medications are never given with the sole aim of sedating patients.
Patients are encouraged and helped to eat and drink by our dedicated kitchen and nursing staff for as long as they can. Food and drink is never denied to patients; if a patient has difficulties swallowing alternatives are considered and discussed with the patient and/or those close to them.
