If you would like to sponsor one or more lights in memory of your loved one/s, please complete the form below. For each light sponsored you will receive a personalised card and your loved one’s name will be included in both our printed and digital Books of Honour.

Please note: The cut-off date to have your loved ones’ names in our Books of Honour that will be displayed at our Light up a Life switch-on service on Sun 7 Dec is Wed 3 Dec. Names remembered from Thu 4 Dec will be printed in a separate Book of Honour displayed at our hospice on Christmas Eve.

Is your donation eligible for Gift Aid?

Using Gift Aid means that we get an extra 25% from the Inland Revenue, helping your donation go further.

I want Wirral Hospice St John’s to treat my donation and all donations I have made in the last four years and all donations I make from the date of this declaration until I notify you otherwise as Gift Aid donations. I understand that if I pay less income tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference.

I am not a UK Taxpayer and cannot Gift Aid this donation

1. To qualify for Gift Aid you must have paid enough UK Income Tax and/or Capital Gains Tax at least equal to the amount that all charities and Community Amateur Sports Clubs will reclaim against my gifts to them in the appropriate tax year, currently 25p for each £1 that you donate. Other taxes such as Council Tax and VAT do not qualify.

2. You can cancel this declaration at any time by notifying Wirral Hospice St John’s.

3. If in the future your circumstances change and you no longer pay tax on your UK Income and Capital Gains equal to the tax claimed by Charities, you can cancel your declaration.

4. If you pay tax at the higher rate you can claim further tax relief in your Self-Assessment tax return.
