Six Steps to Success in End of Life care for Care Homes
Aim of Course:
The aim of this course is to enhance the quality of end of life care by supporting organisational change and developing the roles of staff involved in delivering this care. It focuses on improving end of life care within care homes by embedding the philosophy of palliative care, in alignment with the Care Quality Commission (CQC) Key Lines of Enquiry (KLOEs).
Target Audience: All staff involved in the delivery of end of life care.
Please Note: Attendance is limited to staff employed by A Wirral based care home.
Date & Time of Course:The course is delivered three times a year, in January, May, and September
Duration: The course consists of six sessions delivered over a three-month period
Course Content:
The course consists of six face-to-face taught sessions, complemented by several in-home support sessions delivered directly within the care home setting.
Step 1 Discussions as end of life approaches
Step 2 Assessment, care planning and review
Step 3 Coordination of care
Step 4 Delivery of high quality care in care homes
Step 5 Care in the last days of life
Step 6 Care after death
Communication skills training
Session 1: Introduction & Step to Success (2 hours)
Session 2: Step 1 (4 hours)
Session 3: Step 2 (4 hours)
Session 4: Step 3&4 (4 hours)
Session 5: Step 5&6 (4 hours)
Session 6: Communication Skills (4 hours)
Other Relevant Information:
The programme was developed in the North West of England by the Cheshire & Merseyside Clinical Network and the Greater Manchester, Lancs & South Cumbria Clinical Network with support from National End of Life Care Programme.
Evidence shows the course helps reduce unnecessary hospital admissions at the end of life and improves advance care planning.
Staff who complete the course report increased confidence in discussing death and dying, as well as enhanced knowledge and skills in end of life care.
Number of Places Available:Each cohort can have up to six care homes, with each home nominating two – three representatives to attend the course.
Cost: The course is free of charge, but participating care homes must commit to completing the full programme, including both the taught sessions and in-home support elements.
