Six Steps to Success in End of Life care for Care Homes
Aim of Course:
The aim of the course is to enhance end of life care through facilitating organisational change and supporting staff roles to develop their roles around end of life care. It is aimed at improving end of life care provided by a care home (and its workers) that encompasses the philosophy of palliative care in line CQC key line of enquiries (KLOEs) .
Target Audience: Any Residential or Nursing Home staff on The Wirral. Nurses or Carers from the care home can attend.
Date & Time of Course: The course runs three times a year.
Duration: The course is six sessions over a three-month period.
Course Content:
The course is six face to face taught sessions and several care home support sessions taking place within the home.
Step 1 Discussions as end of life approaches
Step 2 Assessment, care planning and review
Step 3 Coordination of care
Step 4 Delivery of high quality care in care homes
Step 5 Care in the last days of life
Step 6 Care after death
Communication skills training
Other Relevant Information:
The Programme has been developed in the North West of England by the Cheshire & Merseyside Clinical Network and the Greater Manchester, Lancs & South Cumbria Clinical Network with support from the National End of Life Care Programme.
The course is proven to reduce unnecessary hospital admissions at end of life, and improve advance care planning.
Staff report feeling more confident talking about death and dying. Their knowledge and skills around end of life has also increased after attending the course.
Number of Places Available: Six care homes on each cohort. The care home will nominate two representatives to attend the course.
Cost: The course is free, but Care Homes must commit to the whole programme.
