Introduction to End of Life Care Training

Course Aims:

The aim of this course is to equip healthcare professionals with the knowledge, skills, and confidence to deliver high-quality end of life care by:

  • Understanding national and local policies and guidance that shape the development and delivery of end of life care services.
  • Recognising the importance of the North West End of Life Care Model and effectively using prognostic indicators to identify patients who may be in the last year of life.
  • Gaining confidence in using the Wirral End of Life Care Register to ensure appropriate planning and coordination of care.
  • Learning how to support patients through Advance Care Planning (ACP), including identifying their Preferred Place of Care, to ensure care aligns with their values and wishes.
  • Developing the ability to use the Wirral Record of Care document to provide person-centred support during the final days of life.

  • Wirral NHS Community Trust
  • Wirral St John’s Hospice
  • Wirral Care Homes

Duration: Full day

Course Content: The course covers an introduction to recognising people who are in or approaching the last twelve months of life, supporting them with all aspects of care from the end of life care register to advance care planning, to care in the last days of life and bereavement.

The course is suitable for any health professional in a care setting regardless of level of experience.  No previous knowledge is required.
