Day Therapy

Day Therapy takes place from Tuesday to Friday from 10.00 am – 3.30 pm

All referrals for Day Therapy will initially be reviewed in Outpatients’ clinics and an internal referral to Day Therapy will follow.

Once a referral has been received, the Day Therapy team will respond within 7 working days. Patients attending Day Therapy remain under the care of their GP.

Referral of patients to Day Therapy is for a limited time, generally 8 weeks, for rehabilitative care:-

  1. quality of life
  2. maximize and maintain independence
  3. monitoring of symptoms
  4. breathlessness, fatigue & anxiety management workshops
  5. provision of psychological and social support
  6. increased confidence and maintenance of independence
  7. creative and therapeutic activities
  8. social interactions, mutual support and friendship
  9. support for carers and families

Patients attending Day Therapy will be booked to see the nurse for review. Many patients are asked to keep diaries for pain management and/or fatigue.

Once a referral has been received, we will respond within 7 working days from referral. Patients attending Day Therapy remain under the care of their GP.

A Multi-Professional Disciplinary Team (MDT) meeting for Day Therapy is held each Tuesday morning.

Discharge takes place when the identified needs have been met or when the 8 week period of assessment has been completed.

Patients can attend our Evergreen Group while they are attending Day Therapy or once they have been discharged.
